Every time. Took my youngest to her wellness appointment today and forgot her immunization record at home again. Thankfully, our pediatrician has them on file and she's all up to date so no problem this time! And yes... I also forgot the grocery list... So I remembered to get bottled water, but not the water filter to replace the one my toddler gunked up trying to get his paint water clean ^^;;;
